Is health insurance mandatory for residence in Spain?
Not every residence permit in Spain requires private health insurance. We explain exactly when it is mandatory and when it is not.

It is usually mandatory for non-lucrative residence, digital nomad visas, some family reunification cases and certain long-stay visas.
Procedures that usually do not require it
As a general rule, it is not required for ordinary work renewals, employment rooting or many long-term residence renewals, although the specific route must always be checked.
What the accepted policy must look like
When required, the policy must fit the file: no relevant co-payments or deductibles, coverage throughout Spain, enough duration and full medical coverage.
Even when it is not mandatory, having private insurance can make it easier to access specialists quickly and reduce waiting times.
How to assess whether a policy fits your file
When insurance is part of a residence procedure, taking out any policy is not enough. Waiting periods, copayments, coverage in Spain, validity, included insured persons and whether the certificate clearly states the required conditions must be reviewed.
Insurance should also be coordinated with the rest of the file: start date, personal details, address and payment proof. Even a correct policy can raise doubts if it is presented in isolation or with inconsistent details.
Documents worth keeping
Keep the policy, coverage certificate, payment receipts and any communication from the insurer. If the administrative process requires proof of continuity, these records can be decisive.
For renewals or changes of authorization, insurance continuity and the absence of gaps can matter. That is why it is advisable to review expiry dates before the filing date approaches.
